What to Expect:
During your first visit, you'll meet with one of our skilled physical therapists who will conduct a comprehensive evaluation of your condition and formulate a treatment plan with you based on your goals and needs. This assessment may include a review of your medical history, a physical examination, and a discussion of your goals and expectations.
We prioritize open communication, ensuring that you understand your diagnosis and treatment options every step of the way. Your input and feedback are crucial in tailoring a personalized treatment plan that best suits your unique circumstances, promoting optimal recovery and long-term wellness.
What to Bring:
To make the most of your first visit, please remember to bring the following:
· Any relevant medical records, imaging reports, or documentation related to your condition.
· Comfortable clothing such as jogging pants or shorts that allows for movement and access to the affected area.
· Any assistive devices you may use, such as braces, orthotics, walking aids, etc.
· Your insurance information and any required forms or referrals.
Maryland Direct Access for Physical Therapy
In Maryland, you have direct access to physical therapy services, which means you can see a physical therapist without a physician's referral or prescription. This allows for quicker access to care and enables you to begin treatment promptly, without unnecessary delays. However, some insurance requires you to have a referral from your doctor. Please consult with your insurance or call Synergy in Motion PT & Sports Rehab. We are here to help you every step of the way on your path to recovery.

Packages
Evaluation - $150.00 (a $250 value)
· This includes initial evaluation, patient education, exercises/home exercises, manual therapy if necessary, take-home equipment if necessary, TENS/ultrasound/moist heat/ice pack if necessary.
Reevaluation - $120.00 (a $200 value)
· This includes reevaluation (reevaluation is an exam where we take subjective and objective measurements to see if you are improving or not), patient education, exercises/home exercises, manual therapy if necessary, take-home equipment if necessary, TENS/ultrasound/moist heat/ice pack if necessary.
Regular visits - $100.00/visit (a $150 value)
· This visit includes patient education (progress for the week, or lack thereof), exercises/home exercises including balance and proprioceptive exercises, manual therapy if necessary, TENS/ultrasound/moist heat/ice pack if necessary.
Discharge - $100.00 (a $200 value)
· Like the reevaluation, this will include a thorough reevaluation to see if you have met your goals and discharge instructions to keep your chief complaint from coming back. This will also include a home exercise program and self-manual therapy/self-mobilization.
